I'll look into those utilities. Does that mean there is no command line parm to vpinball to remap keys? Is there any list of available command line parameters?
Yep. On the Visual Pinball editor screen, at the top click Preferences > Keys.
Click the button for each key you wish to remap, then press the key that you want to use for that function.

What about the older tables that use a different scheme, such as S to start and A to add a ball? Unlike the newer tables, there's an extra step for each ball on some of those old tables.
