Assuming you want to physically re-create a backglass:
1) If you still have whatever's left of the original, you could send it to BGResto.com in Australia to restore it. See the site for plenty of examples.

Then I believe you'll need to get that printed out on special decal paper applied to a virgin, plain BG. @Skidoo in Canada did something like that late last year. See:

Btw-- my understanding is that they require customers to send them the physical original backglass, even if it's beat to all hell.
I'm not quite sure if that's based on the laws in Oz, for example it might be breaking copyright to physically re-create a backglass they hadn't licensed, whereas it being perfectly legal to restore originals.
Of course it's possible that their policy has changed since then, so you'll need to enquire. And who knows, maybe they have some kind of 'backdoor' service, either way.
