Is there a reference for command line parameters to visual pinball? Is there any way to re-map the left-shift/right-shift and other keys to different keys when you invoke visual pinball?

Do you mean like Bendigo's keycode table, or, Joy to key, or, VP Keys?

All of these are available in the Downloads section of this site (Downloads => Visual Pinball, Flipperless Games => VP Tutorials, support files)
http://www.pinballnirvana.com/UpDownload-req-viewsdownload-sid-6.html

Check them out & if you still have questions, please ask again.

Dan.

I'll look into those utilities. Does that mean there is no command line parm to vpinball to remap keys? Is there any list of available command line parameters?

Yep. On the Visual Pinball editor screen, at the top click Preferences > Keys.
Click the button for each key you wish to remap, then press the key that you want to use for that function.

Ouch, so the only alternative is to edit each pinball file individually? Well, that's good to know at least.

No, the editor Preferences are global, meaning the keys settings affects all tables.

What about the older tables that use a different scheme, such as S to start and A to add a ball? Unlike the newer tables, there's an extra step for each ball on some of those old tables.

Yep, you're right.
The standard keys are programmable in the editor.
Any custom scripted keys will need to be edited in each table's script.
